HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully. MyBenefits is committed to protecting the privacy of your protected health information (PHI) as required by the Health Insurance Portability and Accountability Act (HIPAA).

How we may use and disclose your health information

We may use and disclose your protected health information to administer your health benefits — for treatment coordination, payment of claims, and health care operations such as eligibility, enrollment, care management and quality review. We may also disclose information when required by law.

Your rights regarding your health information

You have the right to inspect and request a copy of your health and benefits records, request corrections, request restrictions on certain uses and disclosures, request confidential communications, and receive an accounting of disclosures. You may also request a paper copy of this notice at any time.

Our responsibilities

We are required by law to maintain the privacy and security of your protected health information, to notify you following a breach of unsecured PHI, and to follow the terms of this notice. We will not use or share your information other than as described here without your written authorization.
